Study Summary: Our long-term goal is to use frailty, functional status and social risk factors to 1) identify at-risk patients, 2) risk-adjust payment models, and 3) design more effective clinical care pathways. Patients in the American College of Surgeons National Surgical Quality Improvement Program (ACS NSQIP) define our cohort; data from diverse sources will be merged to improve predictive modeling
